Official Rules – $200 Amazon Gift Card Survey Giveaway

NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN. VOID WHERE PROHIBITED OR RESTRICTED BY LAW.

1. Sponsor

This giveaway (“Promotion”) is sponsored by Journey Health & Lifestyle (“Sponsor”).

2. Eligibility

Open only to legal residents of the contiguous United States who are eighteen (18) years of age or older at time of entry. Employees of Sponsor, its affiliates, subsidiaries, advertising and promotion agencies, and their immediate family members or persons in the same household are not eligible. Void in Alaska, Hawaii, Puerto Rico, and where prohibited.

3. Promotion Period

The Promotion begins at 8/13/2025 12:00 am EST and ends at 9/13/2025 11:59 am EST (“Promotion Period”). Sponsor’s computer is the official timekeeping device.

4. How to Enter

  • Survey Entry: Complete and submit the designated survey during the Promotion Period. Limit one (1) survey entry per person.

  • Alternate Free Entry: To enter without completing the survey, send an email to [hello@shopjourney.com] with your full name, email address, and the subject line “$200 Gift Card Giveaway – AMOE” during the Promotion Period. Limit one (1) email entry per person; duplicate entries will be disqualified. Both entry methods have an equal chance of winning.

5. Prize

One (1) winner will receive a $200 Amazon.com Gift Card (Approximate Retail Value: $200). Gift card subject to Amazon.com’s terms and conditions; no cash or prize substitutions permitted. Prize is non-transferable.

6. Winner Selection and Notification

Winner will be selected by random drawing from all eligible entries received during the Promotion Period, to take place within thirty (30) days of the survey’s launch date. Winner will be notified by email within five (5) business days of drawing. If winner does not respond within seven (7) calendar days after notification, an alternate winner may be selected.

7. Odds of Winning

Odds depend on the total number of eligible entries received.

8. Taxes

All federal, state, and local taxes associated with receipt or use of prize are the winner’s sole responsibility.

9. Limitation of Liability

By participating, entrants agree to release and hold harmless Sponsor, its affiliates, and agencies from any liability arising out of participation or receipt/use of the prize.

10. Privacy

Information provided will only be used for administering this Promotion, unless otherwise stated in Sponsor’s Privacy Policy here.

11. General Conditions

Sponsor reserves the right to cancel, suspend, or modify the Promotion due to fraud, technical problems, or other factors beyond Sponsor’s reasonable control.

To request the name of the winner, send an email to hello@shopjourney.com with the subject line: Survey Giveaway Winner List, within 30 days of the end of the Promotion Period.
