On its own, obesity can put a significant burden on virtually every system in the body. It's been linked with high blood pressure, type 2 diabetes, cardiovascular disease, sleep apnea, and other chronic conditions. Older adults may be particularly impacted, since being overweight and having obesity can amplify the natural physical changes that come with aging, says Peminda Cabandugama, M.D., a specialist in obesity medicine and endocrinology at Cleveland Clinic in North Olmstead, OH.
"Excess weight and obesity can have large direct and indirect effects on older adults," he explains. "This ranges from a greater amount of visceral fat—the kind stored in the abdomen that wraps around vital organs, and which increases cardiometabolic risks—to issues with increased frailty and worsening balance."
Although the effects of obesity can be serious, even a small amount of weight loss can pivot your health in the right direction, says Dr. Cabandugama. For example, the Obesity Action Coalition notes that losing just 5% to 10% of your body weight can have significant beneficial effects on your health.

Excess weight increases the mechanical load on muscles, joints, and the cardiovascular system during every movement, and that can make even everyday activities feel more daunting. "The extra weight with obesity makes mobility difficult in a way that often leads to reduced movement overall," says Karena Wu, D.P.T., a board-certified clinical specialist in orthopedic physical therapy and the owner and clinical director of ActiveCare Physical Therapy in New York City. "This reduced movement can accelerate the aging process and increase functional decline."
For example, you may find you're feeling fatigued or short of breath while climbing a short flight of stairs, getting up from a chair, or walking across a parking lot, according to Partha Nandi, M.D., a clinical associate professor of medicine at Michigan State University in East Lansing. He notes that excess body fat can create chronic inflammation in the body, making it harder for muscles to use energy efficiently and causing fat to accumulate in muscles and even bone marrow. That inflammation and reduced muscle strength can make routine tasks feel exhausting.
Age-related conditions can exacerbate this effect, he adds. Issues like osteoarthritis and sarcopenia (loss of skeletal muscle mass) are both commonly seen with aging, and obesity can worsen their progression. As that happens, it can profoundly affect mobility. Other chronic conditions may also be negatively affected by obesity's impact on mobility, adds Rick Douglass, D.P.T., a doctor of physical therapy and chief clinical officer at FYZICAL Therapy and Balance Centers in Sarasota, FL.
"Reduced movement can lead to deconditioning of the cardiovascular and respiratory systems, as well as loss of muscle strength, balance, and endurance," Douglass notes. "Over time, this decline in physical conditioning can make everyday activities more difficult, further limiting mobility and independence."
The Connection Between Obesity, Joint Pain, and Arthritis
The relationship between obesity and osteoarthritis is both mechanical and biological, and that distinction matters, says Dr. Nandi. "On the mechanical side, every pound of excess body weight adds roughly four pounds of force on the knee joint with each step," he says. "Over years and decades, that additional stress accelerates the wear on cartilage."
Meanwhile, the type of chronic, low-grade inflammation triggered throughout the body with obesity affects joint tissue and can lead to joint degeneration, even in joints that don't bear significant weight, he adds. That can lead to pain, stiffness, and progression of arthritis, which can all contribute to reduced physical activity.
How Obesity Affects Balance and Fall Risk
Excess weight changes the body's center of gravity, affecting how you move and respond to unexpected changes in balance, according to Dr. Nandi. Combined with the muscle weakness that often accompanies obesity, this can significantly compromise stability.
"People may unconsciously alter their gait, further reducing balance," he explains. "Reaction time, such as the ability to catch yourself when you start to fall, tends to be slower as well. The result is a meaningful increase in fall risk." Dr. Nandi adds that older adults with obesity have a higher risk of more severe falls "because excess weight increases the forces involved and because underlying health conditions can complicate recovery."
As concerns about falling and balance increase, people often cut back even further on movement because it feels safer to remain sedentary than risk injury, says Dr. Cabandugama. Over time, this reduction in activity can contribute to further weight gain, muscle loss, and reduced joint function, which can worsen balance issues and heighten fall risk.
Signs Your Weight Is Impacting Mobility
Indications that your weight is affecting your mobility can be both subtle and overt. "The first signs are activities that used to require little to no effort are becoming more difficult and taxing," says Douglass. "Activities like getting in and out of your car, standing up from a low chair, and even rolling over in bed require more effort as you gain weight and you may find yourself limiting your activity to avoid this strain."
Other indications that obesity is impacting mobility include:
- Avoiding stairs altogether whenever possible (taking the elevator or escalator)
- Difficulty with flexibility, such as bending over to pick up an object or tie your shoes, or reaching an item on a high shelf
- Experiencing increased frequency of falls or nearly falling more often
- Pausing often to rest or catch your breath
- Preference for walking shorter distances
- Reduced participation in hobbies or activities, particularly if they include movement
- Reduced walking speed
- Reliance on outside support, such as handrails, walls, and furniture
- Shortness of breath when walking or climbing stairs
- Struggling to get up from seated without pushing on a chair's arms
- Taking wider, shorter steps to maintain your center of gravity
- Uncontrolled movements that rely on momentum, such as dropping into a chair instead of lowering slowly
Ways to Improve Mobility and Stay Independent
It's Catch-22: Obesity can impede mobility, but physical activity is an important part of maintaining a healthy weight, says Wu. One workaround is to focus on physical activities that don't put extra strain on the joints. "Water-based exercises are wonderful for increased mobility and decreased joint stress when carrying excess weight," she says. "The water adds buoyancy and can help reduce the perception of pain. It also allows an individual to perform exercises with fewer restrictions."
Seated activities like using a stationary bike or doing chair exercises can also help improve mobility without the increased compression in the weight-bearing joints, she adds. "The bike is a non-weight bearing cardiovascular exercise that can improve circulation and lower extremity mobility," she says. "Chair exercises allow the majority of your body weight to be supported while the extremities can be mobile to perform exercises without the added stress and balance required to stand."
Strength and balance programs can also be helpful, but it's important to consult with a physical therapist or personal trainer who has experience working with those who have excess weight or obesity. That way, you can progress gradually and safely.
Mobility Aids That Support Safer Movement
In addition to working more physical activity into your schedule, mobility aids can be a boon for building confidence, reducing fall risk, conserving energy, and participating in daily life, says Dr. Nandi.
When you're living with obesity, be sure you select a mobility device that supports extra pounds, Dr. Nandi emphasizes. Not all pieces of equipment are safe at any amount of weight loading. If your weight is right at the cutoff for the capacity of a device, always choose the model with greater weight capacity.
Devices that can support your mobility with obesity include:
- Walkers
- Rollators
- Electric wheelchairs
- Scooters
- Lift chairs
- Grab bars
Focusing on a Healthy Weight Helps Mobility as You Age
There is nothing easy about losing weight, but keep in mind that dropping just 5% of your body weight can make a measurable improvement in your overall health. As you begin to shed pounds, you'll have less joint pain and stiffness, more energy throughout the day, better sleep quality, and improved balance.
Older adults struggling with weight and reduced mobility should start with an evaluation by a physical therapist to develop an individualized plan focused on strength, balance, and functional movement, Douglass says. "Even with gradual weight loss, mobility can improve significantly through targeted exercise and increased activity," he adds. "The focus should be on moving more efficiently, reducing pain or fatigue, and maintaining independence in daily activities."
