How Obesity Leads to Joint Pain (and Ways to Get Relief)
You're probably not surprised to learn that carrying extra weight puts more pressure on your joints, which can be challenging as you get older and joint health is affected by other factors. Most notably, aging brings natural wear and tear that involves cartilage breakdown, reduced lubrication in the joints, and muscle weakness.
But the increased pressure and forces on your body from obesity are only part of the story when it comes to weight and joint pain, says Tracy Norfleet, M.D., a specialist in obesity medicine at BJC Health in St. Louis, MO.
Understanding the impact of carrying extra pounds on your joints—and in turn, how both affect your mobility—is the first step in taking back control of your health.

How Obesity Affects Your Joints
"There is a complex interplay between biomechanical, inflammatory, and metabolic mechanisms with joint pain," says Dr. Norfleet, who is also a spokesperson for The Obesity Society, the leading scientific authority in obesity research and evidence-based care, and chairman of the advocacy committee for the Obesity Medicine Association. "While excess weight does increase the physical forces placed on joints, the relationship between obesity and joint pain is far more than just additional pressure."
A quick science lesson: Adipose tissue, or the fat that your body stores, is not an inert substance beneath the skin. Instead, it's an endocrine organ on its own, and when there's too much accumulation of it, it causes inflammatory chemicals to be released. These inflammatory substances contribute to joint degradation, says Dr. Norfleet. Meanwhile, metabolic issues associated with obesity, like elevated blood sugar, also affect the joints, contributing to osteoarthritis-related joint pain, she adds.
When joint pain goes up, mobility naturally goes down. The good news? Research shows that losing just 5% to 10% of your body weight can have a notable effect on restoring mobility and improving your health.
Why Joints Become More Vulnerable With Age
Like many people, you may have noticed changes in your bones, muscles, and joints as you've gotten older. That's normal: According to the National Library of Medicine, many people experience these common effects of aging:
- Cartilage in the joints begins to wear away
- Joints become stiffer and less flexible as fluid in the joints decreases
- Lean body mass decreases, caused by a loss of muscle tissue
- Loss of bone mass or density increases, especially in women after menopause
- Minerals may deposit in and around some joints, particularly in the shoulder
- Muscles are less able to contract due to changes in the tissue and nervous system
- The body's ability to maintain and repair joint cartilage slows down, exacerbating changes
All of these changes make joints more vulnerable to additional stressors like excess weight, which places greater mechanical stress on joints in the spine and the knees, says Dr. Norfleet.
How Excess Weight Puts Stress on the Joints
The more weight there is on the body, the more stress there will be on the joints because the load they are carrying increases, says Rick Douglass, D.P.T., a doctor of physical therapy and chief clinical officer at FYZICAL Therapy and Balance Centers in Sarasota, FL. This can be especially noticeable on joints such as the knees, spine, hips, and ankles, where extra pounds can accelerate the wear and tear on cartilage that protects those joints, he says.
"Since obesity is also associated with chronic low-grade inflammation throughout the body, this inflammatory state can contribute to joint pain and may worsen the degeneration of cartilage and other joint structures," Douglass says. "Adding to the problem, as joint pain increases, people often become less active, which can lead to muscle weakness, reduced joint support, and further limitations in mobility and function."
As a result, obesity can contribute to both the development of joint pain and the progression of some chronic conditions through a combination of increased mechanical stress and inflammation, he explains.
Conditions Related to Obesity and Joint Pain
Obesity itself is considered a chronic disease, but it is also known to raise the risk for numerous other conditions. These are a few of the other common health issues tied to obesity and joint pain.
Osteoarthritis
This form of arthritis is the most common joint-related health condition linked to obesity, according to Dr. Norfleet, with research showing that up to two-thirds of the elderly obese population experience osteoarthritis of the knee. This is when the cartilage between your joints wears away, and you lose cushioning that protects the joint.
Signs of osteoarthritis in the joints include an ache or soreness during or after movement, trouble moving a joint through its full range of motion, mild swelling, and popping or cracking when using the joint.
Gout and Obesity
Another issue related to excess weight and joint pain is gout, which happens when a waste product in your blood called uric acid builds up and forms tiny crystals inside the joint. These crystals cause inflammation and intense pain and swelling in your joints. It can feel like your joints are on fire or that there are tiny needles inside the joint. As gout progresses, moving the affected joints may be more difficult, leading to reduced mobility. The effects of aging can cause gout to linger, according to Dr. Norfleet.
"Our kidneys become less efficient at clearing uric acid as we age," she says. "Obesity increases the production of uric acid, while aging decreases the kidneys' ability to remove it, which is why the risk of gout climbs with both obesity and aging combined."
Rheumatoid Arthritis
Rheumatoid arthritis is a chronic autoimmune and inflammatory disease. Although excess body fat does not cause rheumatoid arthritis directly, since fat tissue produces inflammatory substances, it can make the disease more active and harder to control, Dr. Norfleet says. Rheumatoid arthritis can cause painful swelling in the joints, most commonly in the small joints in the hands, wrists, and feet. "The good news is that losing weight lowers the inflammation, which is a key driver of joint damage and pain," she adds.
How Joint Pain Affects Mobility and Independence
When joint pain becomes chronic, a natural reaction is to avoid walking, exercising, or participating in activities that increase the pain, says Lindsy Jackson, D.P.T., a doctor of physical therapy and owner and director of Movement Managed PT in Littleton, NH. "Over time, reduced activity can contribute to weakness, decreased endurance, balance impairments, and difficulty performing activities of daily living," she says.
The impact of joint pain is more than physical, adds Dr. Norfleet. Limitations on movement lead to social isolation and feelings of vulnerability and helplessness. In turn, that may contribute to depression, which has been linked to more intense pain perception for some people. "It becomes a cycle that's hard to break, and it starts with avoiding physical activity because of pain," she says. "That's why addressing this is so important."
How to Manage and Relieve Joint Pain Caused by Obesity
Alleviating joint pain requires a multi-strategy approach, says Jackson. Lifestyle habits that support weight management and physical activity are important. Pain management options may provide temporary relief, and mobility aids are an important intervention that give you the option to move when it feels OK, and rest when it doesn't.
Weight Management
It's hard to oversell the importance of weight loss for easing joint pain. Research has found that losing just 10 pounds relieves 40 pounds of pressure on your knees, for example. Physical activity, strength training, sleep health, and an overall healthy diet can significantly influence how joints function throughout life, says Jackson.
Pain Management Options
When joints are stiff, aching, and painful, it's helpful to address things directly with pain management, says Douglass. That can include:
- Heat therapy. Research suggests thermotherapy (heating pad, warm compress, or warm bath) may provide short-term improvements in pain and physical function for people with knee osteoarthritis, although the quality of the evidence varies.
- Cold therapy. An ice pack or cold compress can temporarily numb pain that is caused by inflammation. A 2025 systematic review found that cryotherapy can reduce pain in people with knee osteoarthritis.
- Anti-inflammatory medication. Nonsteroidal anti-inflammatory drugs, or NSAIDs, such as ibuprofen, naproxen, and diclofenac, can reduce both inflammation and joint pain in the short-term, studies show.
- Physical therapy. Low-impact aerobic activity, strengthening exercises, and range-of-motion movements can decrease pain while improving strength, balance, and joint function. A large 2025 systematic review in the British Journal of Medicine found that aerobic exercise was particularly effective for improving pain in people with knee osteoarthritis.
Mobility Aids May Help
The expression "motion is lotion" holds true in managing joint pain, but sometimes, you need an assist. Mobility aids provide stability and balance support so you can move with more confidence. Some options include:
- Walkers
- Rollators
- Power chairs
- Scooters
When you have obesity, make sure you choose a device with appropriate weight capacity (read the product guidelines to be sure). Not all mobility aids are safe at any amount of weight loading. The best choice is a product with a weight capacity that is above (not right at) your weight.
When to See a Doctor
Don't wait to seek help until joint pain becomes so intense and frequent that it is limiting your mobility. "The sooner you see the doctor, the better," says Dr. Norfleet. "If joint pain begins to interfere with daily activities or lasts beyond a few weeks, you should see a healthcare provider. For people living with obesity, it is important to not assume that joint pain is just from the weight." Furthermore, seek a doctor's evaluation right away if a joint suddenly becomes red, hot, and swollen, she adds.
